Refactor Plan skill: what it does and how to install it

What it does. Investigates the repository without editing, then documents the current and target states, affected files, dependencies, phased implementation steps, verification commands, risks and rollback actions. It pauses after presenting the plan and requests confirmation before changes begin.

When to use it. For multi-file refactors that need careful sequencing and scope control. If the request is too ambiguous to plan safely, it asks clarifying questions instead of editing.
