About this skill
What it does. Creates either a Nygard-style ADR for architecture-level choices or a lightweight dated decision-log entry. It captures the decision, rationale, alternatives, consequences and references, requires negative consequences, and treats accepted records as append-only, with later entries used for supersession.
When to use it. Use after a choice is made or an existing decision is superseded, including decisions buried in chat or review comments. Do not use it while a decision is still being argued; use design documentation instead.
